What Is the Prostate?
The prostate is a small gland, about the size of a chestnut, that is part of the male reproductive system. It is located in front of the rectum, just below the bladder, and surrounds the urethra—the tube that carries urine out of the body.
The prostate's main function is to produce part of the seminal fluid, which mixes with sperm to form semen.
A healthy adult prostate weighs approximately 20 to 25 grams and measures about 4 cm (1.6 in) wide, 3 cm (1.2 in) high, and 2 cm (0.8 in) thick.

Dr. Wolfgang Willem L.
Graduated from the Autonomous University of Nuevo Leon, Dr. Willem is a board-certified urologist with more than 33 years of experience and a long-standing commitment to innovation in advanced urologic technology. He is a member of the Mexican College of Urology and the Mexican Society of Urology, as well as a founding member of urological associations in Baja California, San Luis Rio Colorado, and Sonora. He currently serves as Director of Clínica San Agustín, a medical center specializing in advanced urologic procedures.
Dr. Iván Robles
Board-certified urologist and graduate of the Autonomous University of Nuevo Leon (UANL), with advanced fellowship training in Minimally Invasive and Robotic Surgery. He is an active member of Mexico's National System of Researchers (SNII), under the Ministry of Science, Humanities, Technology and Innovation (SECIHTI). He also serves as Co-Editor of the Mexican Journal of Urology (RMU), is the author of more than 35 peer-reviewed scientific publications, and has received multiple national and international awards and recognitions for his contributions to clinical research.


Dr. Gilberto Paz
Board-certified urologist and graduate of the University of Guadalajara. He is a member of several national and international urological associations and serves as a Professor of Clinical and Theoretical Urology at the Autonomous University of Baja California (UABC), Mexicali campus.
Dr. Paz is a pioneer in the use of botulinum neurotoxins for the treatment of male sexual dysfunction and is currently developing a patented therapeutic approach in this field. He has extensive experience in clinical research protocols and is recognized for his pioneering work in the management of urethral complications associated with radical prostate cancer treatments. His broad institutional experience spans clinical practice, research, and academic medicine.
Dr. Juan Sierra
Board-certified urologist and graduate of the Autonomous University of Guadalajara. He is a member of several national and international urological associations and a pioneer in Functional Urology (Urodynamics) in northwestern Mexico.
Dr. Sierra is an investigator in prostate cancer clinical research protocols and has performed more than 500 ultrasound-guided prostate biopsies, making him one of the region's most experienced specialists in this procedure. He has advanced training in the implantation of penile prostheses and previously served as a Professor of Medicine at Universidad Xochicalco, Mexicali campus. He also brings extensive experience in institutional healthcare and academic medicine.

How Does It Work?
Imagine sunlight passing through a magnifying glass. The rays are concentrated into a single focal point, generating intense heat exactly where it is needed.
Focal One works in much the same way by using High-Intensity Focused Ultrasound (HIFU) to precisely target and destroy prostate tumor tissue, millimeter by millimeter, through controlled thermal energy (coagulative necrosis), while preserving the surrounding healthy tissue.
HIFU (High-Intensity Focused Ultrasound) Technology
Focal One HIFU is an innovative, minimally invasive treatment option for patients with intermediate-risk localized prostate cancer. This focal therapy uses High-Intensity Focused Ultrasound (HIFU) to generate precisely targeted thermal energy that destroys cancerous tissue within the prostate—without surgery, incisions, or radiation. Available in northwestern Mexico through our team of urologists, Focal One offers an effective treatment alternative for patients seeking to minimize the side effects commonly associated with more invasive therapies, such as radical prostatectomy or radiation therapy.
Unlike conventional treatments, Focal One HIFU targets only the areas affected by cancer while preserving as much healthy surrounding tissue as possible. By combining multiparametric MRI with real-time ultrasound imaging, physicians can accurately identify the treatment area and deliver highly precise focal therapy. This advanced technology provides exceptional accuracy, effectively destroying cancerous tissue while protecting the structures responsible for critical functions such as urinary continence and sexual function.
The goal of Focal One HIFU is to achieve effective cancer control while minimizing the impact on the patient's quality of life. The procedure is performed under general anesthesia on an outpatient basis, allowing patients to return home the same day. What Is the HIFU Focal One Procedure Like?

Patient Preparation:
Before the procedure, the patient undergoes diagnostic tests such as multiparametric MRI and a prostate biopsy to accurately identify the cancerous areas. This information allows the physician to carefully plan a personalized treatment with maximum precision. The procedure is performed under general anesthesia, so the patient remains asleep throughout the treatment.

Patient Positioning:
Once under anesthesia, the patient is positioned lying on their side, allowing the physician to access the prostate through the rectum. This approach enables the HIFU transducer to precisely deliver focused ultrasound energy to the targeted treatment area while minimizing the impact on surrounding healthy tissue.


Procedure:
The physician inserts a small, cylindrical HIFU transducer into the patient's rectum. Through this device, High-Intensity Focused Ultrasound (HIFU) is delivered precisely to the cancerous areas that were previously identified during treatment planning.
Image-Guided Precision: Focal One integrates multiparametric MRI with real-time ultrasound imaging, allowing the physician to clearly visualize the treatment area and accurately target only the cancerous tissue while preserving the surrounding healthy prostate.
Cancer Ablation: The focused ultrasound waves generate controlled thermal energy at specific locations within the prostate, destroying cancer cells without the need for surgery, incisions, or radiation. Throughout the procedure, the physician controls the robotic system to ensure the treatment is delivered with exceptional precision and effectiveness. Once the treatment is complete, the transducer is removed, and the patient is transferred to the recovery area.
Procedure Duration: The treatment typically lasts between 1 and 3 hours, depending on the size and location of the area being treated.

Recovery:
Because Focal One HIFU is performed as an outpatient procedure, patients are typically able to return home the same day. Recovery is generally rapid, and most patients can resume their normal daily activities within just a few days.
